Clinica Sierra Vista, a non-profit corporation begun in December 1971, was created to provide health care to poor, migrant farmworkers who harvest the lush agricultural fields in the southern San Joaquin Valley. Today, Clinica Sierra Vista serves a geographically dispersed, low-moderate-fixed income, ethnically diverse, frontier-rural-urban-migrant-homeless patient population. It has grown to be one of the largest comprehensive migrant/community health center systems in the state.

Goals

  • Medical, dental, respite care and case management services are provided to the homeless population county-wide at specialize facilities.
  • Mobile van visits emergency shelters, low-income motels and homeless encampments

Behavioral Health Services

  • Adult Services provide evaluation and brief therapy support for individuals 18 and over in the communities of Arvin, Delano, Frazier Park, Lamont, McFarland and Weedpatch.
  • Children’s Services provide individual, family and group counseling, case management, psychiatric evaluation and medication support in Arvin, Frazier Park and Lamont.
  • C.H.A.T. Program – intervention and treatment services for victims of abuse.
  • Substance Abuse Services in Delano and McFarland.

Clinical Support Services

  • Cancer Detection Program: Free annual clinical breast exam, mammogram, cervical cancer screening, and case management to qualified women over forty.
  • Diabetes Collaborative: Provides individual and group education and case management for diabetic patients.
  • Family Planning: Including free exams and contraceptive services for eligible uninsured individuals.
  • Material Child Outreach Program: Provides case management, prenatal care and support, childbirth education, home and hospital visits.
  • Medial Nutrition Therapy: Upon referral for health center patients.
  • Medically Vulnerable Infants Program: Case management and home visiting program for eligible babies released from the Neonatal Intensive Care Unit.

Women, Infants & Children (WIC)

  • Operating 21 WIC locations throughout Kern County.
  • WIC provides nutrition education, breast feeding support and food vouchers for pregnant and post-partum women, infants and children up to five years of age.  Visit WIC’s web site at www.csvwic.org

HIV / AIDS

  • We provide specialized infectious disease and primary care services to HIV/AIDS and Hepatitis C patients.
  • Kern Lifeline Project: Case management services, individual and small group behavioral health counseling, advocacy and emergency assistance to people with HIV/AIDS and their families.
  • Clinica Sierra Vista offers confidential and anonymous HIV testing, as well as education and prevention services for the general public, especially targeting at-risk populations.

Health Promotion / Outreach

  • Black Infant Health: Comprehensive, community based effort to reduce infant mortality by serving at-risk pregnant and parenting African-American women and assuring that they have access to maternal and child health services.
  • Family Support Programs: Neighborhood-based services utilizing case management, home visitation, and multidisciplinary team conferences, located in family resource centers in Lamont, Mojave and Southeast Bakersfield.
  • Nurturing Infant Awareness: Reduce African-American infant mortality with prenatal case management, transportation to medical appointments and support groups.

Adolescent Family Services

  • Adolescent Family Life Program and CalLearn: Case management and supportive services for pregnant and parenting teens and their families.
  • Community Challenge Grant: Community effort discouraging teen pregnancy by utilizing teen panel presentations and the Baby Think It Over Program.
  • Health Education Outreach Programs : Family Life Education for junior high age youth and younger as well as the information and Education Program in community and school settings for 13-18 year olds.

Becoming a Patient

  • Making Appointments - You can make an appointment at any of Clinica Sierra Vista’s health centers by calling the location you wish to visit and scheduling an appointment. Health center hours vary by location, so please call the location nearest you for the days and hours of operation.
  • Same-Day Appointments (Walk-Ins) - Clinica Sierra Vista also accepts same-day appointments at all of our health centers, although your wait time may be longer, since patients with scheduled appointments will be seen first.
  • What to Bring With You - When visiting the health center for the first time, you will need to bring proper identification, which may include a social security card, valid driver’s license, birth certificate or another form of official identification. If available, you should also bring your Medi-Cal card, Medicare card or private insurance card. If uninsured, you will also need to bring a pay stub, an unemployment check stub, a recent tax return or a letter from your employer in order to prove your family income.
  • Insurance - Clinica Sierra Vista accepts Medi-Cal, Medicare, most private insurance, most HMO plans and cash. We have Medi-Cal outreach workers and representatives from Healthy Families at our health centers that will assist patients with enrollment in these programs.
  • Uninsured - For the uninsured, we accept payment using a sliding fee scale, based on family size and family income.
